About this skill
Problem to Solve
Patients with thyroid nodules or PTC often face the same ultrasound finding interpreted in different contexts: TIRADS, microcalcifications, and vertical orientation can raise malignancy concern during screening, yet are not simple exclusion criteria during active surveillance. The skill aims to place ultrasound reports, lab values, and doctor recommendations into the correct phase, reducing the risk of misunderstanding small 3.6mm nodules, non-indicated FNA, surgery, or excessive imaging as standard care.
How It Works
It evaluates in two phases:
- Phase 1: Screening assessment: reads nodule size, morphology, TIRADS class, lymph nodes, and lab indicators, then checks whether FNA thresholds from C-TIRADS or ACR TI-RADS are met.
- Phase 2: Active surveillance: for confirmed or highly suspected PTC, assesses TTD, LTD, location, and follow-up frequency to discuss whether active surveillance may replace immediate surgery.
Its core is active correction: when the user mentions nodule size, treatment advice, or test results, it checks ten dimensions: FNA indication, surgery indication, size-action match, lymph nodes, lab tests, follow-up interval, imaging choice, age factors, and pathology interpretation. Outputs use ✅, ⚠️, and ❌, cite thresholds and sources, and frame advice as something to discuss with the treating physician.
Boundaries and Notes
This skill is health education and medical-visit reference only, not a diagnosis. Final medical decisions must be made by the treating physician. Seek immediate care for dysphagia, dyspnea, or hoarseness. Content reflects guidelines current as of 2026-04-23, and standards may change.
Use Cases
- After receiving a thyroid ultrasound, check whether the TIRADS class meets C-TIRADS FNA thresholds.
- After PTC diagnosis, evaluate TTD, LTD, and location for active surveillance instead of surgery.
- When FNA or surgery is recommended, verify indication against size, lymph nodes, and lab markers.
- During follow-up planning, assess whether 6-month initial and 12-month stable intervals are reasonable.
Best For
- Thyroid nodule patients: want to know whether TIRADS and microcalcifications mandate FNA.
- Diagnosed PTC patients: want to assess whether small lesions can be actively monitored instead of surgery.
- Follow-up users: want to confirm intervals, calcitonin/TSH, and imaging are not excessive or missed.
- Medical educators: want to cite guideline thresholds when explaining FNA indications and surveillance limits.
