8D reports often fail because the format is easy but the content is hard: D2 lacks quantitative data, D4 names a vague root cause, and D5/D6 actions are not tied to verification. This skill turns the report into a structured interview, keeping authors from starting in front of a blank template.

How It Works

The skill begins with D0 scenario anchoring, inferring industry, issue type, severity, and role from keywords: line/batch/process suggests manufacturing, while version/deployment/logs suggests software. After confirmation, D1 through D8 present questions in batches, each with one or two context-aware suggestions. Users can accept a suggestion, accept it with edits, provide custom answers, or mark a stage as skipped. Quality gates check key fields: D2 needs What/When/Where/How Many data; D4 requires a 5-Why or fishbone path plus validation; D5 actions must address the root cause; D6 must include verification evidence or a plan. Reports can be generated as SQE-style .docx using python-docx, with a title bar, reference fields, and a D0-D8 table, or exported as Markdown for internal sharing and version control.

Boundaries

Dynamic suggestions depend on the facts provided by the user. Vague answers trigger follow-up questions, and unfinished stages are marked as pending. If the issue does not require an 8D, the skill may suggest A3 or One-Pager instead. It is useful for quality, SQE, supplier corrective action, and internal issue reviews, but it does not replace root-cause validation, accountability, or formal sign-off.
